This event will bring together former patients\, caregivers\, care team members\, and community partners to celebrate accomplishments and provide an opportunity for patients to connect with other stroke survivors and reunite with members of their care team. \nAttendees will be able to: \n\nMeasure progress with an assessment\nVisit with community partners to explore activities and resources that are available\nEnjoy snacks and refreshments\n\n  \nFriends\, family\, and caregivers welcome. \nThis event will be held rain or shine.
